Here are some references for this topic:

Montse Stanley, Handbook of Knitting, pg 112

June Hemmons Hiatt, Principles of Knitting, pg 68.

I've looked through Vogue Knitting and they cover the right but not the left slanting increase. Maybe I am missing something. Does anyone have additional references for this.

Elsebeth Lavold's book Viking Patterns for Knitting has the method, with illustrations, for left/right on both knit and purl fabric (p112-3 in my book).

Anyone got a great website for this information (especially the left-slanting lifted increase)... a good picture or video would be fabulous!

Nancie Wiseman's Knitter's Book of Finishing Techniques calls it Raised Increase, but its the same thing. pages 32 & 33. She provides both illustrations of increase in progress and photos of both directions completed.
